The US Family Plan (USFP) is a health care option under TRICARE, a health program for active-duty service members, retirees, and their families. It is specifically designed to provide civilian health care services to eligible military families and retirees in designated regions of the United States.
JACC is a State-funded program that provides a broad array of in-home and community-based services to individuals age 60 and older who meet clinical eligibility for nursing home level of care
TBI services refer to Traumatic Brain Injury (TBI) services, which are specialized programs, interventions, and support systems designed to assist individuals who have experienced a traumatic brain injury. These services aim to help individuals recover, adapt, and improve their quality of life following an injury.
Respite services for families with children, youth, and young adults under age twenty-one 21with developmental disability (DD). Agency Hired Respite (AHR): This service is provided to families who want a respite worker who is recruited, trained, and employed by a qualified agency to provide social and recreational experiences to youth in or out of their homes in the community. AHR is 6 limited to up to 240 units (60 hours) per 90 days. Families can utilize time as needed within the 90-day authorization.
The Division of Developmental Disabilities (DDD) program in New Jersey provides services and support to individuals with developmental disabilities to help them live as independently as possible. It is managed by the New Jersey Department of Human Services (DHS) and offers a variety of programs designed to enhance the quality of life, promote community inclusion, and support both individuals and their families

The CCN plan in New Jersey is a program designed to provide eligible Veterans with access to healthcare services from community providers when the VA cannot provide the care directly. This plan is part of the Department of Veterans Affairs efforts to ensure Veterans receive timely and high-quality healthcare, even outside VA facilities.
In New Jersey, the VA Community Care Network is managed by a third-party administrator, Optum (part of UnitedHealth Group), which handles provider networks, claims, and customer service for community care.
